Nature of modifying agreement
1. All types.	(1) A heading in the following form of words - "Agreement modifying a Hire Agreement and regulated by the Consumer Credit Act 1974". (2) Where the credit is being secured on land the words "secured on" followed by the address of the land shall be inserted at the end of the heading.
Parties to modifying agreement
2. All types.	(1) The name, postal address and, where appropriate, any other address of the owner. (2) The name, postal address and, where appropriate, any other address of the hirer.
