(1) A credit information provider shall include on its forms for the making of qualifying credit applications a notice stating that this Act requires the provision of information to the Bank relating to qualifying credit applications and qualifying credit agreements for entry on the Register.
(2) The Bank may, with the consent of the Minister, make regulations specifying the content and form of notices required by subsection (1).
